The role requires RN licensure and 1+ year of acute care experience; hospice/home health preferred. Full-time with on-call duties, competitive benefits, and salary range $80,000-$96,000.

Qualificações

• RN license from an accredited nursing program.
• 1+ year of acute care experience; hospice/home health preferred.
• Ability to assess, plan, and educate patients and families.

Responsabilidades

• Conduct initial and ongoing assessments to determine hospice needs.
• Develop individualized care plans with patient and family.
• Administer medications and treatments per orders.
• Lead end-of-life conversations and coordinate care with the team.
