St. Croix Hospice is seeking a Registered Nurse to provide day-to-day hospice patient care under the direction of the RN Case Manager. Focus on comfort, quality of life, and planning of care in home settings.

Responsibilities include medication administration, care coordination, and family education. Graduate of an accredited nursing program with an active RN license is required. The role requires 100% travel in assigned area, strong assessment skills, and adherence to HIPAA and

• Graduate of an accredited school of nursing.
• Must hold a current RN license in state of practice.
• Ability to pass background, consumer report and drug test.

Responsabilidades

• Assess patient and family needs and develop Plan of Care.
• Administer medications and treatments as prescribed.
• Document patient care and maintain accurate records.
• Coordinate Plan of Care with interdisciplinary team and community partners.
• Provide patient and family education and support.
