Job summary

St. Croix Hospice is seeking a Registered Nurse to deliver compassionate, patient-centered care to hospice patients in home or facility settings.

You will implement the Plan of Care under the direction of the RN Case Manager, administer medications, document care, and collaborate with the interdisciplinary team to ensure comfort and quality of life. Ideal candidates have an accredited nursing degree, an active RN license in applicable states, and the ability to pass background checks.

Qualifications

• Graduate of an accredited school of nursing required. Associate degree required.
• Must have a current Registered Nurse (RN) license in state(s) of practice.
• Must pass background check, consumer report and drug test.
• Valid State Driver’s License with reliable vehicle.

Responsibilities

• Provide day-to-day hospice patient care under the direction of the RN Case Manager.
• Administer medications and treatments as prescribed and maintain records.
• Complete comprehensive patient assessments and contribute to Plan of Care revisions.
• Maintain accurate clinical notes and communicate patient needs to the physician.
• Assist patients with activities of daily living and ensure a safe home environment.
• Collaborate with the hospice interdisciplinary team and coordinate care with community partners.
