Job summary

Care New England is seeking a Hospice Registered Nurse to manage a caseload of patients as part of an interdisciplinary team. You will assess, plan, and coordinate care to promote comfort and quality of life.

You will collaborate with physicians, social workers, and home health staff, educate families, document care, and ensure smooth transitions. RI RN license, CPR, and a Bachelor's in Nursing are required.

At least 1 year hospice experience preferred.

Qualifications

• RI Registered Nurse license required
• Bachelor’s Degree in Nursing or related field preferred
• At least 1 year of clinical nursing experience (hospice preferred)
• Current CPR certification
• Valid driver’s license and automobile liability insurance
• Excellent written and verbal communication skills
• Strong clinical judgment and problem-solving abilities

Responsibilities

• Manage a caseload of patients as part of an interdisciplinary team
• Assess, plan, and coordinate care while monitoring and adjusting treatment as needed
• Develop comprehensive individualized care plans and document updates
• Coordinate care and referrals across disciplines and specialties
• Educate patients and families on home-based care and symptom management
• Ensure continuity of care through clear communication with patients, families, and providers
• Identify opportunities for care improvement and participate in quality initiatives
