Job summary

Care New England is seeking a Hospice Registered Nurse to manage a patient caseload within an interdisciplinary team. You will assess, plan, and coordinate care while monitoring and adjusting treatment to promote the best outcomes.

The role requires RI RN licensure, CPR, and at least 1 year of clinical nursing experience, with hospice experience preferred. Excellent communication and clinical judgment are essential.

Qualifications

• RI Registered Nurse license is required
• Strongly preferred Bachelor’s Degree in Nursing or related field
• At least 1 year of clinical nursing experience (hospice preferred)
• Current CPR certification
• Valid driver’s license and automobile liability insurance
• Excellent written and verbal communication skills
• Strong clinical judgment and problem-solving abilities

Responsibilities

• Perform comprehensive assessments and develop personalized care plans
• Coordinate care and referrals across disciplines and specialties
• Monitor patient progress and adjust care to achieve optimal outcomes
• Communicate with patients, families, and providers to ensure continuity of care
• Educate patients and caregivers on managing conditions at home
• Document clinical notes and updates with accuracy and clarity
• Identify opportunities for care improvement
