Qualifications

• Valid Rhode Island LPN license is required.
• At least 1 year of progressive clinical experience (home health or acute care preferred).
• Current CPR certification.
• Valid driver’s license and auto insurance.
• Strong clinical, communication, and documentation skills.

Responsibilities

• Provide direct nursing care, including treatments and wound care.
• Monitor patient conditions and report changes to the care team.
• Develop and adjust individualized care plans.
• Collaborate with nurses, therapists, and other professionals.
• Educate patients and caregivers to promote independence and self-care.
• Document all care provided and patient updates.
• Participate in team meetings and interdisciplinary coordination.
