Job summary

Care New England Health System in Rhode Island is seeking a Registered Nurse specializing in end-of-life care within the VNA Home Health & Hospice network. The RN will provide compassionate, holistic care to patients nearing the end of life, supporting families and addressing physical, emotional, and psychosocial needs.

The role collaborates with an interdisciplinary team to develop and implement individualized care plans, manage pain, and ensure ethical and legal compliance throughout care

Qualifications

• Bachelor’s degree in Nursing.
• RI licensure required.
• Minimum of one year’s job-related experience.

Responsibilities

• Conduct thorough assessments of patients' physical, emotional, and spiritual needs.
• Develop individualized care plans based on assessments, patients' wishes, and medical directives.
• Collaborate with interdisciplinary teams to ensure care plans address comprehensive needs.
• Administer medications and treatments to manage pain and alleviate distressing symptoms.
• Monitor and evaluate the effectiveness of interventions to ensure patient comfort.
• Provide emotional support to patients and their families, addressing fears, concerns, and emotional distress.
• Facilitate open and honest conversations about patients' feelings, wishes, and end-of-life goals.
• Educate patients' families about the dying process and what to expect in the final stages.
• Provide guidance on caregiving techniques, emotional support, and coping strategies.
• Maintain accurate and up-to-date electronic health records (EHR) reflecting patient assessments, interventions, and outcomes.
• Ensure adherence to ethical guidelines and legal regulations in end-of-life care.
