Paycom - ATS is seeking a Registered Nurse Case Manager to support hospice patients at home or in facilities in the Jim Thorpe, PA area. The role emphasizes skilled nursing care, care coordination, and end-of-life support in partnership with the interdisciplinary team.

Standard hours are Monday–Friday, with potential on-call responsibilities and territory coverage within roughly an hour of Jim Thorpe. A robust benefits package accompanies the position.

Qualifications

• Active RN license in good standing.
• Hospice or home health experience preferred.
• Strong clinical assessment and care coordination skills.
• Ability to work independently in the field.
• Strong communication and documentation skills.
• Experience using HCHB (Homecare Homebase) preferred.

Responsabilités

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing care in accordance with hospice standards and physician orders.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Educate patients and families on symptom management, disease progression, and end-of-life care.
• Document assessments, interventions, and care plans accurately and timely in the EMR HCHB (Homecare Homebase).
• Participate in interdisciplinary team meetings and care planning.
• Support after-hours patient needs as part of an on-call rotation when applicable.

Overview

The Care Team is seeking a Registered Nurse Case Manager to join our growing team! The Registered NurseCase Managersupports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end‑of‑life support in partnership with the interdisciplinary team.

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Territory: Up to a 1 hour radius of Jim Thorpe, PA
• RN Case Managers may participate in an on-call rotation with additional pay
• Some branches offer dedicated on-call staff, resulting in backup on-call coverage only
