The Care Team Home Health & Hospice in Lehigh Valley, PA is seeking an experienced RN Case Manager to deliver skilled nursing care within hospice guidelines and coordinate patient-centered plans. Qualifications

• Active RN license (state-licensed)
• Hospice or home health experience preferred
• Strong clinical assessment and care coordination skills
• Ability to work independently in the field
• Strong communication and documentation skills
• Experience using HCHB (Homecare Homebase) preferred

Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care in accordance with hospice standards and physician orders
• Coordinate care with physicians, caregivers, and interdisciplinary team members
• Educate patients and families on symptom management, disease progression, and end-of-life care
• Document assessments, interventions, and care plans accurately and timely in the EMR HCHB
• Participate in interdisciplinary team meetings and care planning
• Support after-hours patient needs as part of an on-call rotation when applicable

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Up to a 1-hour radius around Lehigh Valley, PA
• RN Case Managers may participate in an on-call rotation with additional pay
• This branch offers dedicated on-call staff, resulting in backup on-call coverage only

Field Role Requirements

• Active driver's license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
