The Care Team Home Health & Hospice in Fishers, IN is seeking a Registered Nurse Case Manager to deliver skilled hospice nursing and coordinate care for patients at home or in facilities. You will work with the interdisciplinary team to ensure high-quality end-of-life care and patient comfort.

Standard hours Monday–Friday, from 8:00 AM to 4:30 PM, with potential on-call rotation for additional compensation. Territory covers up to a 1-hour radius of Indianapolis, IN.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.

Responsabilités

• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing care in hospice standards per physician orders.
• Coordinate care with physicians, caregivers and interdisciplinary team.
• Educate patients and families on symptom management and end-of-life care.
• Document assessments and care plans accurately in HCHB.
• Participate in interdisciplinary team meetings and care planning.
• Support after-hours patient needs during on-call rotation when applicable.
• Complete visits, admissions and interventions with timely documentation.

Overview

The Care Team is seeking a Registered Nurse Case Manager to join our growing team! The Registered Nurse Case Manager supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team.

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Territory: Up to a 1 hour radius of Indianapolis, IN (Mostly no the eastern and southern sides of the city)
• RN Case Managers may participate in an on-call rotation with additional pay
• Some branches offer dedicated on-call staff, resulting in backup on-call coverage only

Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized plans of care
• Provide skilled nursing care in accordance with hospice standards and physician orders
• Coordinate care with physicians, caregivers, and interdisciplinary team members
• Educate patients and families on symptom management, disease progression, and end-of-life care
• Document assessments, interventions, and care plans accurately and timely in the EMR HCHB (Homecare Homebase)
• Participate in interdisciplinary team meetings and care planning
• Support after-hours patient needs as part of an on-call rotation when applicable
• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
