Ascend Hospice & Palliative Care PA in Lancaster, PA is hiring an RN Case Manager to join our hospice team. The position involves on-call coverage, case management, and patient visits from Monday to Friday 8:00 AM–5:30 PM.

Responsibilities include initial patient/family assessments, re-evaluating nursing needs, care plan development, coordination with the healthcare team, counseling on end-of-life needs, administering nursing procedures, communicating with physicians, and supervising Hospice

Competenze

• Must have a State Certified RN license.
• 2 years' experience as a professional nurse, hospice experience preferred.
• Must have a valid Driver's license and reliable transportation.
• Must be willing to travel.
• Strong verbal and written communication skills.

Hours: M-F 8AM-5:30PM
