Ascend Hospice & Palliative Care PA is hiring an RN Case Manager in Lancaster, PA (York/Lancaster Area). The role involves initial assessments, care planning, and collaboration with the healthcare team to ensure continuous, well-coordinated care.

Requirements include Associate's Degree in Nursing, an RN license, at least 2 years nursing experience with hospice preferred, a valid driver's license, ability to travel, and strong communication skills.

Qualifications

• Must have an RN license (state certified).
• 2 years' experience as a professional nurse; hospice experience preferred.
• Must have a valid driver's license and reliable transportation.
• Must be willing to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete an initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluates the patients nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated.
• Counsels the patient and family in meeting end of life needs.
• Initiates appropriate preventative and palliative nursing procedures.
• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition.
• Supervises the Hospice Aides.

Hours: M-F 8AM-5:30PM

The RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
