Job summary

Trinity Health At Home – Grand Rapids is seeking a Home Health Registered Nurse to deliver skilled nursing care in patients' homes and develop individualized care plans. You will administer medications, educate patients and families, and support recovery and independence.

This role emphasizes 1:1 patient relationships, clinical autonomy, flexible scheduling, and meaningful work with a supportive nurse-led leadership team.

Qualifications

• Graduate of an approved Nursing education program; BSN preferred.
• Current RN licensure in Michigan.
• Minimum of one year professional RN experience.
• Medical Surgical experience preferred.
• Home health or Hospice experience a plus.
• Strong computer and technology skills required.
• Must have current Driver’s license and reliable transportation.

Responsibilities

• Deliver skilled nursing care in patients' homes.
• Create individualized care plans for each patient.
• Administer medications as prescribed.
• Educate patients and families to support recovery and independence.
• Help patients maintain independence and quality of life in the home.

Minimum Qualifications

• Graduate of an approved Nursing education program, BSN preferred.
• Current Registered Nurse licensure in the State of Michigan.
• Must have a minimum of one (1) year professional registered nursing experience.
• Medical Surgical Experience preferred
• Home health or Hospice experience is a plus
• Strong computer and technology skillsrequired.
