Job summary

Trinity Health At Home – Grand Rapids (Michigan) is seeking a Home Health Registered Nurse (RN) to deliver skilled nursing care in patients' homes, create individualized care plans, administer medications, and educate patients and families. This full-time role offers top-of-license practice, strong 1:1 patient relationships, and a nurse-led team with supportive leadership.

No on-call requirements, flexible scheduling, and meaningful outcomes.

Qualifications

• Graduate of an approved Nursing education program; MI licensure required.
• Must have a minimum of one (1) year professional registered nursing experience.
• Home health or Hospice experience is a plus.

Responsibilities

• Deliver skilled nursing care in patients' homes and create individualized care plans.
• Administer medications and educate patients and families.
• Support recovery and independence through comprehensive care and coordination.
• Maintain accurate documentation and effective communication with care team.

Minimum Qualifications

• Graduate of an approved Nursing education program, BSN preferred.
• Current Registered Nurse licensure in the State of Michigan.
• Must have a minimum of one (1) year professional registered nursing experience.
• Medical Surgical Experience preferred
• Home health or Hospice experience is a plus
• Strong computer and technology skills required.
• Must have current Driver’s license and reliable transportation.
