Job summary

Buckeye Hospice and Palliative Care Services is seeking a skilled RN to provide comprehensive nursing assessments and implement/revise care plans for hospice patients across multiple locations in Ohio. This role involves coordinating with caregivers and interdisciplinary teams to ensure responsive, high-quality care.

The ideal candidate will have 1–4 years of hospice or home health experience, an active RN license, valid driver's license, and participate in on-call rotations.

Responsibilities

• Performs initial and comprehensive nursing assessments of patient needs.
• Implements and reevaluates the plan of care to meet nursing needs and goals.
• Communicates patient progress to patients, caregivers and other disciplines; directs and educates direct care staff to deliver quality care.
• Assesses and cares for hospice patients in nursing facilities and home settings.

Job Responsibilities

• Provides the initial assessment and comprehensive nursing assessment of patient needs.
• Implements the plan of care and reevaluates and revises the plan of care relative to the patient’s nursing needs to provide appropriate, comprehensive and responsive goals and interventions.
• Communicates the patient’s progress with the patient/caregiver and other disciplines as well as directs, supervises and instructs direct care staff members and nonprofessional hospice aide staff to deliver quality care and coordinates delivery of care services.
• Assessment and care of Hospice patients in nursing facilities and home settings.

Job Requirements

• RN License is REQUIRED.
• Valid Drivers License and reliable transportation that is insured in accordance with Ohio state requirements.
• Minimum of 1 to 4 years as a RN working with hospice services or home health agency.
• On-Call Rotation.
• EOE Drug Free Workplace
