Job summary

Compassus is seeking a Hospice Registered Nurse (RN) Case Manager in Indiana (Kokomo area). You’ll support patients and families, coordinate in-home care, and ensure compassionate, quality end‑of‑life services.

You will collaborate with the care team, educate families on care needs, and help manage symptoms with dignity and comfort. Indiana RN license and hospice experience preferred, valid driver’s license required.

Qualifications

• RN license in the state of practice.
• Two years of experience as a registered nurse, hospice experience preferred.
• Valid driver’s license.
• Current CPR and BCLS certification.
