Job summary

Ascension at Home together with Compassus is seeking a Hospice Registered Nurse / RN Case Manager to support patients and families in providing compassionate, dignity-centered care in a home setting.

In this role, you will build trusting relationships with patients and families, empower caregivers with practical education on care, and collaborate with the care team to ensure comfort and peace during challenging times.

Qualifications

• State RN license in the state where you work.
• Minimum two years of registered nurse experience; hospice experience preferred.
• Current driver’s license.
• Current CPR and BCLS certification.
