LilyCare Hospice in Albuquerque, NM is hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery. You will manage a caseload, perform assessments, and deliver comprehensive care in patients’ homes and facilities.

You will coordinate pain and symptom management with the physician and leadership, participate in IDG meetings, educate families, and ensure accurate EMR documentation while rotating on-call duties.

Qualifikationen

• Current RN license in New Mexico or a compact state.
• Two years of RN experience required; hospice, home health, or case management experience preferred.
• Current CPR certification.
• Valid driver’s license.

Aufgaben

• Serve as the Hospice Registered Nurse Case Manager for a caseload in private homes and facilities — complete assessments, plans of care, and routine visits.
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director.
• Coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy.
• Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care.
• Complete timely EMR documentation and participate in the RN Case Manager on-call rotation.

Duties and Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities — complete assessments, individualized plans of care, and routine visits
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director
• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy
• Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care
• Complete timely, accurate EMR documentation and participate in the RN Case Manager on-call rotation

Compensation and Schedule

The Hospice Registered Nurse Case Manager role pays $90,000 - $105,000/year based on experience. Full-time, Monday–Friday day schedule with a rotating on-call assignment. Territory is based out of Albuquerque, NM.
