Job summary

Brighton/Suncrest Hospice is seeking a Registered Nurse to visit patients in homes, SNFs, and assisted living facilities. You will be part of an interdisciplinary team, offering psychosocial assessments and coordinating care.

The role includes Case Management, Admissions, or Resource duties with a Tuesday–Saturday, 8:00 AM–4:30 PM schedule and travel across the service area. The RN will work autonomously yet stay closely connected with nurses, social workers, chaplains, and CNA leadership to

Qualifications

• Current RN licensure (in good standing) in the state(s) of practice.
• Ability to collaborate daily with an interdisciplinary team.
• Will travel across the service area with reliable transportation.
• Availability Tue-Sat 8am–4:30pm.

Responsibilities

• Provide care to a group of patients visiting their homes or facilities.
• Participate as part of the interdisciplinary team, focusing on psychosocial assessments.
• Work in Case Management, Admissions, or Resource role.
