Job summary

Brighton/Suncrest Hospice seeks a Resource Nurse (RN) to provide patient care across homes, SNFs, and assisted living facilities within the service area. You will join an interdisciplinary team, offering psychosocial assessment expertise and linking patients to community resources.

This role includes field visits, admissions support, and collaboration with healthcare professionals, with a Monday–Friday schedule from 11:00 am to 7:00 pm.

Qualifications

• RN licensure in good standing (GA)

Responsibilities

• Provide care and services for a group of patients visiting their residences, which may include private homes, SNFs, and assisted living facilities.
• Act as a contributing member of the interdisciplinary team, offering expertise on psychosocial assessments and community resources.
• Aids in field visits, admissions, and other needs of the team.
