Job summary

Brighton/Suncrest Hospice is seeking a Registered Nurse to provide patient care across homes and facilities. You will be part of an interdisciplinary team, performing psych-social assessments and coordinating with community resources.

The position runs Monday through Friday from 12:00 PM to 8:00 PM, with opportunities for bonus and growth as the national company expands its footprint.

Qualifications

• Current Registered Nurse (RN) licensure in good standing in state(s) of practice.
• Collaborates daily with nurses, social workers, chaplains, and CNA leadership.
• Able to travel across service area with reliable transportation.
• Proficient in using an electronic medical records system.

Responsibilities

• Provide care and services for a group of patients who reside in homes or facilities.
• Visit patients Monday–Friday 12p-8p as part of an interdisciplinary team.
• Assess psychosocial needs and connect families with community resources.
