Job summary

Qualifications

Active RN license in Utah.
Strong clinical assessment and critical-thinking skills.
Excellent communication and interpersonal skills.
Ability to work independently and with an interdisciplinary team.
Familiarity with electronic documentation and email.

Responsibilities

Complete hospice nursing visits and comprehensive patient assessments.
Provide skilled nursing and comfort-focused care.
Monitor changes in condition and respond to patient needs.
Educate and support patients, families, and caregivers.
Coordinate care with the hospice team.
Maintain accurate and timely clinical documentation.
Make independent clinical decisions within scope of practice.
