Job summary

Rocky Mountain Care Hospice in Salt Lake City seeks a compassionate Hospice Registered Nurse (RN) to join our team on a 7-on / 7-off schedule. You will perform visits and comprehensive assessments, delivering patient-centered end-of-life care with consistent routines.

Responsibilities include nursing care, family education, interdisciplinary coordination, and precise documentation. Utah RN license required; hospice/home health experience preferred. In-person work location.

Qualifications

• Current, active RN license valid to practice in Utah.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaborate with an interdisciplinary team.
• Comfortable using electronic documentation and email.
• Hospice, home health, or related nursing experience preferred.

Responsibilities

• Perform hospice nursing visits and comprehensive patient assessments.
• Provide skilled nursing and comfort-focused care.
• Monitor changes in patient condition and respond to needs.
• Educate and support patients, families, and caregivers.
• Coordinate care with the interdisciplinary hospice team.
• Maintain accurate and timely clinical documentation.
• Make independent clinical decisions within scope of practice.
