Job summary

Rocky Mountain Care Hospice is seeking a compassionate Hospice Registered Nurse (RN) to join our Salt Lake City team on a 7-on / 7-off schedule. You will perform hospice nursing visits, complete patient assessments, and provide comfort-focused care to patients at home.

Strong assessment and communication skills are essential for supporting patients, families, and caregivers. Join a dedicated interdisciplinary team and contribute to exceptional end-of-life care.

Qualifications

• Active Utah RN license required.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and with an interdisciplinary team.
• Hospice, home health, or related nursing experience preferred.

Responsibilities

• Complete hospice nursing visits and patient assessments.
• Provide skilled nursing and comfort-focused care.
• Monitor changes in condition and respond to patient needs.
• Educate and support patients, families, and caregivers.
• Coordinate care with the interdisciplinary hospice team.
• Maintain accurate and timely clinical documentation.
• Make independent clinical decisions within your scope of practice.
