Job summary

Northshore Home Health Care is seeking a Hospice Registered Nurse (RN) to provide compassionate patient care in a home-based setting. You will apply established nursing theories and document care to support reimbursement while coordinating physical care with patients, families, physicians, and the hospice team.

You will communicate effectively with patients, families, and staff, and participate in IDT meetings and quality reviews.

Qualifications

• Current RN licensure and graduation from an approved nursing program.
• At least two years of clinical nursing experience, with one year in hospice preferred.
• Knowledge of home health and hospice operations and adherence to nursing standards and agency policies.
• CPR certification and 12 hours of annual continuing education.
• Flexibility for travel, weekends, evenings, and holidays.
