Job summary

Northshore Home Health Care is seeking an Experienced Hospice Registered Nurse (RN) to provide compassionate, high-quality care to patients facing terminal illness. You will apply nursing theories in patient care and document care to meet reimbursement guidelines.

You will coordinate physical care with patients, families, physicians, and the hospice team, communicate effectively with all stakeholders, and participate in IDT meetings and quality reviews.

Qualifications

• Current RN licensure in the practice state.
• Completion of an approved nursing program with at least two years of clinical experience; hospice experience preferred.
• Knowledge of home health and hospice care frameworks, nursing practices, and reimbursement guidelines.
• 12 hours of annual continuing education and CPR certification.
