As a Hospice RN / RN Case Manager, you will:

• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior, maintaining primary control and professional management of each patient.
• Serve as the main liaison between physicians, patients, families, and the hospice team.
• Provide direct patient care and support to families, managing a reasonable caseload with compassion and trust.
• Educate caregivers on patient care to empower them.
• Collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to ensure comfort, peace, and dignity.
• Assess patient and family needs, and assist in developing and implementing care plans.
• Maintain accurate clinical records for all patients.
• Participate in quality improvement and performance programs.

About You

Requirements for Hospice Registered Nurse / RN Case Manager:

• Compassionate, kind, and empathetic towards patients and families.
• Comfortable providing care in the patient's home environment.
• Flexible and composed during crises.
• RN license in the relevant state or eligibility to obtain one.
• At least 1 year of nursing experience in hospice, home health, ICU, geriatrics, or oncology is preferred.
• Additional experience in hospital settings, ER, critical care, or related fields is a plus.
• Valid driver’s license and insurance.
• Current CPR certification.
