As a Hospice RN / RN Case Manager, you will:
  1. Report directly to the Administrator, Executive Director, or Patient Care Manager and maintain primary control and management of each patient.
  2. Serve as the primary liaison between the physician, patient/family, and hospice team.
  3. Provide direct patient care to hospice patients and supportive care to their families, fostering trusting and compassionate relationships.
  4. Empower caregivers through education on patient care.
  5. Collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to deliver comfort, peace, and dignity.
  6. Assess patient and family needs and assist in developing and implementing care plans.
  7. Maintain all clinical records for assigned patients.
  8. Participate in quality assessment and performance improvement programs.
About You

Hospice Registered Nurse / RN Case Manager Requirements:

  • Compassionate, kind, and empathetic towards patients and families.
  • Comfortable providing care in the patient's home environment.
  • Flexible and able to maintain composure during crises.
  • RN license in the state of employment or eligibility to obtain one.
  • At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
  • Additional experience in hospital settings such as Med Surg, ER, ICU, or critical care is advantageous.
  • Valid driver’s license and auto insurance.
  • Current CPR certification.
