Transitions Hospice is seeking a Care Coordinator to grow referrals and deliver hospice care within the Fort Wayne region. You will be the face of the organization in your territory, building strong relationships with physicians, nursing facilities, hospitals, discharge planners, and case managers to ensure access to exceptional hospice care.

The role requires a background in healthcare sales, 3+ years of sales experience, and a Bachelor’s degree preferred.

Formación

• Bachelor’s degree preferred or higher in a relevant field.
• 3+ years of sales experience in healthcare.
• Proven track record building customer relationships that drive results.
• Excellent communication, negotiation and PR skills.

Responsabilidades

• Generate referrals and admissions from current and new customers within your territory.
• Establish and maintain relationships with physicians, nursing homes, hospitals, discharge planners, and case managers.
• Represent Transitions Hospice professionally with referral sources and the community.
• Deliver education to accounts and serve as a trusted resource on hospice care.
