Job summary

Atlantic Visiting Nurse in Morristown, NJ is seeking an experienced Registered Nurse to join our Care Access Center as a Home Care & Hospice Referral Specialist. You will assess referrals, determine the appropriate level of care, coordinate admissions, and work with physicians, discharge planners, and care teams to ensure timely, compassionate transitions from hospital to home.

Qualifications

• Registered Nurse with licensure to practice in New Jersey.
• Experience in hospice or home care and care coordination.
• Ability to educate patients and families on services and care plans.

Responsibilities

• Conduct comprehensive clinical assessments to determine eligibility for Home Care or Hospice services.
• Review medical documentation to ensure referrals meet criteria and regulatory requirements.
• Meet with patients and families to educate on Home Care and Hospice services, benefits, and care philosophy.
• Collaborate with physicians, case managers, discharge planners, and interdisciplinary teams to coordinate transitions of care.
• Complete timely referrals and documentation within Epic.
• Coordinate equipment and services needed prior to discharge.
• Participate in interdisciplinary care planning and clinical case discussions.
• Maintain knowledge of home care regulations, hospice standards, reimbursement guidelines.
