Job summary

365 Hospice LLC is seeking an experienced Hospice Clinical & Community Liaison for the Greater Pittsburgh area. You will develop and maintain referral relationships with physicians, hospitals, SNFs, and home health agencies, educating them about hospice services and identifying suitable patients.

The role is a relationship-driven healthcare sales position requiring independent territory development and consistent growth in admissions, with base salary and uncapped commissions.

Qualifications

• Previous hospice or home health sales experience is REQUIRED.
• Experience in healthcare sales, business development, marketing, or community liaison work within hospice/home health is preferred.

Responsibilities

• Develop and grow hospice referral relationships within the assigned territory.
• Educate physicians, hospitals, SNFs, and home health agencies about hospice services.
• Generate qualified referrals and drive census growth.
• Conduct regular face-to-face visits with referral sources.
