Friendly Faces Senior Care seeks an experienced Hospice Marketer / Community Liaison to drive referrals and expand community partnerships. The role emphasizes relationship building, field outreach, and educating partners about hospice services and eligibility.

The ideal candidate will have established healthcare relationships, understand the hospice referral process, and be comfortable traveling to build a robust referral network and increase census.

Qualifications

• 2+ years in hospice, home health, healthcare, or senior-care marketing/business development preferred.
• Established healthcare referral network and hospice marketing experience preferred.
• Proven history generating healthcare referrals and developing professional referral relationships.
• Knowledge of hospice services, Medicare hospice eligibility, and the referral process preferred.
• Strong sales, presentation, networking, communication, and relationship-building skills.
• Self-motivated with ability to spend significant time in the field.
• Ability to organize and manage a territory independently.
• Strong follow-up skills and attention to detail.
• Valid driver's license, reliable transportation, and ability to travel throughout the territory.
• Maintain confidentiality and comply with HIPAA and applicable regulations.

Responsibilities

• Develop and maintain referral relationships with hospitals, physicians, SNFs, assisted living, home health, and other healthcare professionals.
• Generate qualified hospice referrals through consistent business development and outreach.
• Conduct in-person calls, presentations, facility visits, networking events, and educational meetings.
• Educate referral partners about hospice services, eligibility, benefits, and the referral process.
• Identify new referral opportunities and strategies to increase census and market penetration.
• Maintain regular communication and follow-up with existing and prospective referral sources.
• Build relationships with discharge planners, case managers, social workers, physicians, admins, and nursing directors.
• Coordinate with admissions and clinical teams to ensure smooth referrals and admissions.
• Track activities, referrals, conversions, and sources using CRM or reporting systems.
• Participate in healthcare associations, community events, and other business development activities.
• Monitor market activity and identify opportunities to strengthen competitive position.
• Meet weekly/monthly activity, referral, admissions, and business-development goals.
• Represent the hospice organization professionally and ethically within the community.
