UnitedHealth Group's Amedisys-backed opportunity seeks a Clinical Hospice Liaison to build referrals and manage relationships within a defined territory. You will implement sales plans, track referrals in CRM, and help move patients from referral to admission, traveling roughly half the time.

Required qualifications include a bachelor's degree with 1+ year in healthcare or related fields and an active RN or LPN license (MSW/LCSW/PT/OT/SLP/PA also acceptable).

Competenze

• Bachelor's degree with 1+ year healthcare or related experience.
• Current, unencumbered RN or LPN license in the state of service (MSW/LCSW/PT/OT/SLP/PA acceptable).
• Hospice or healthcare business development experience preferred.

Mansioni

• Achieves admissions and payor-mix goals and meets target account goals.
• Maintains referrals record with physicians, hospitals, SNFs and case managers.
• Documents sales calls daily in CRM with pre/post-call notes.
• Assists referral-to-admission process for care centers.
• Travels 50% or more within the territory.
• Develops and maintains client relationships in a competitive market.
• Prepares business plans and prioritizes accounts per market plan.
• Responds to customer complaints promptly.
