Job summary

Heart to Heart Hospice is seeking a Community Education Representative to drive hospice referrals and business development in the local market. You will call on physicians, hospitals, and senior living facilities to promote our services and Heart to Heart Hospice House, while coordinating with the team on admissions and community outreach.

The role emphasizes territory planning, customer relationships, and adherence to Medicare guidelines, with a focus on timely, autonomous execution and ongoing

Qualifications

• 1–3 years of progressive hospice/home health/healthcare sales experience preferred.
• Knowledge of business management and the Medicare Hospice Benefit.
• Excellent time management and organizational skills; ability to work autonomously.
• Valid driver’s license and reliable transportation with current auto liability insurance.

Responsibilities

• Call on physicians, area hospitals, SNFs, assisted living facilities or other senior living settings to promote hospice referrals and Heart to Heart Hospice House.
• Collaborate with hospice-eligible patients and loved ones during admission with Hospice House staff.
• Develop and implement territory business plans and manage sales time/resources to meet goals.
• Establish and maintain public relations to increase brand awareness and community understanding of the organization and services.
• Utilize CRM software and other tools to manage accounts and performance.
• Perform other duties as required; ensure compliance with laws, Medicare regulations and policies.
