Job summary

Bane Care Management, LLC seeks a motivated Hospice Liaison to support referral development, community outreach, and patient admissions for hospice services. You will build relationships with providers, hospitals, SNFs, and other partners to raise awareness and ensure smooth admissions.

The role requires travel to facilities, excellent communication and organizational skills, and a commitment to HIPAA compliance.

Qualifications

• Bachelor's degree from an accredited four-year college or university.
• Minimum of two years of sales, marketing, business development, or related experience.
• Strong communication, relationship-building, organizational, and interpersonal skills.
• Proficiency in Microsoft Office and general computer applications.
• Valid driver's license and ability to travel regularly.
• Familiarity with hospice, and post-acute care

Responsibilities

• Build and maintain relationships with healthcare providers, hospitals, skilled nursing facilities, assisted living communities, and other referral sources.
• Promote services through community outreach, educational presentations, and professional networking.
• Coordinate referrals, admissions, assessments, and related documentation.
• Collaborate with interdisciplinary teams to support care coordination and customer service.
• Maintain compliance with organizational policies, HIPAA, and applicable regulations.
• Attend required meetings and training, follow safety procedures, and report concerns.
• Travel regularly to healthcare facilities, community partners, and patient locations.
