Job summary

Gentiva is seeking a Hospice Physician to deliver compassionate, goal-directed care to hospice patients. You will collaborate with interdisciplinary teams, oversee regulatory compliance, and guide quality improvement efforts to improve patient outcomes.

This is a notification-only role; the Hospice Physician will invoice for services and travel as required, with 15-25% travel based on program needs. A board-certified background with hospice/palliative medicine experience is preferred.

Qualifications

• Doctorate in allopathic or osteopathic medicine from an accredited medical school.
• Board certification in ABMS or AOA-BOS, preferably Hospice/PC medicine or related.
• Current unrestricted medical license in practicing state and valid NPI/DEA if applicable.

Responsibilities

• Certify and recertify hospice eligibility per Medicare and regulations.
• Oversee face-to-face visits and home consultations for complex cases.
• Provide 24/7/365 on-call physician coverage coordination.
• Maintain timely medical documentation for regulatory reviews.
• Participate in IDG meetings to coordinate care and med management.
• Collaborate with leadership on policy development and QAPI initiatives.
• Travel as needed to program sites to support patient care.
