Job summary

Gentiva is seeking a Hospice Physician to ensure compassionate, goal-directed care for hospice patients. You will collaborate with interdisciplinary teams, oversee regulatory compliance, and guide quality improvement efforts to enhance patient outcomes.

The Hospice Physician will invoice Gentiva for services rendered; travel 15-25% may be required. You will participate in IDG meetings, supervise face-to-face visits, and provide 24/7 on-call coverage.

Qualifications

• Must hold Doctorate in medicine (MD/DO).
• Board certification in a recognized specialty, preferably primary care.
• Current, unrestricted medical license in the practicing state.
• Valid NPI and DEA registrations for controlled substances.
• Hospice/Palliative Medicine board cert or Hospice Medical Director cert preferred.
• Current Medicare PECOS and Medicaid certification if required.

Responsibilities

• Certify and recertify hospice eligibility in a timely manner.
• Supervise home visits and complex cases.
• Provide 24/7/365 on-call coverage coordination.
• Maintain accurate medical documentation for regulatory reviews.
• Participate in IDG meetings to coordinate patient care.
• Monitor medication prescribing and utilization.
• Collaborate on policy development and QAPI initiatives.
• Serve as community liaison to enhance hospice partnerships.
• Support staff and community education programs.
