Job summary

Gentiva Health Services, Inc. seeks a Hospice Physician to ensure compassionate, goal-directed care for hospice patients and to lead care with interdisciplinary teams. You will oversee regulatory compliance and guide quality improvement to improve patient outcomes.

This role involves collaborating with leadership on policy development, participating in education programs, and traveling to meet program needs while supporting a high standard of medical practice in hospice care.

Qualifications

• MD/DO degree with board certification in a related field.
• Unrestricted medical license and applicable state credentials.
• NPI and DEA registrations for controlled substances.
• Board-certified in Hospice and Palliative care preferred.
• Ability to travel 15–25% as required.
• CPR and TB clearance completed.

Responsibilities

• Certify and recertify hospice eligibility per Medicare and regs.
• Supervise home visits and coordinate with IDG teams.
• Provide 24/7/365 on-call coverage coordination.
• Maintain timely medical documentation and support reviews.
• Engage in policy, QA, and performance improvement initiatives.
• Act as liaison to enhance hospice partnerships with providers.
• Support staff and community education programs.
