Job summary

Gentiva Health Services, Inc. seeks a Hospice Physician to ensure compassionate, goal-directed hospice care, collaborating with interdisciplinary teams and guiding quality improvement while upholding medical standards.

This is a non-employee notification; the Hospice Physician will invoice Gentiva for services rendered and participate in regulatory compliance and performance improvement efforts to enhance patient outcomes.

Qualifications

• Doctorate in allopathic or osteopathic medicine from an accredited school.
• Board certification in a recognized specialty (ABMS or AOA-BOS).
• Current and unrestricted medical license in practicing state.
• Valid NPI and DEA registration for controlled substances.
• Hospice/Palliative Medicine board certification or Hospice Medical Director Cert preferred.
• PECOS/Medicaid certification if required.

Responsibilities

• Certify and recertify hospice eligibility per Medicare and regulations.
• Conduct and supervise home visits for complex cases.
• Provide 24/7/365 on-call coverage coordination.
• Maintain accurate medical documentation and support regulatory reviews.
• Participate in IDG meetings to coordinate care and med management.
• Monitor pharmacy utilization and appropriate prescribing.
