Job summary

Gentiva seeks a Hospice Physician to ensure compassionate, goal-directed care for hospice patients. You will lead medical decisions, supervise home visits, and support quality improvement across the program. This is a notification-only arrangement with invoicing for services rendered.

You will collaborate with interdisciplinary teams, ensure regulatory compliance, and participate in education for staff and partners. Travel up to 25% may be required.

Qualifications

• Doctorate in allopathic or osteopathic medicine is required.
• Board certification in a recognized specialty is required or preferred.
• Active, unrestricted medical license in the practicing state.
• NPI and DEA registration for controlled substances.
• Hospice and Palliative Medicine board cert or Hospice Medical Director cert preferred.
• Medicare/Medicaid enrollment experience may be required.
• Ability to travel 15–25% based on program needs.
• CPR and TB clearance are required.

Responsibilities

• Certify and recertify hospice eligibility in compliance with Medicare and regulations.
• Conduct and supervise home visits for complex cases.
• Provide 24/7/365 on-call coverage coordination.
• Maintain accurate medical documentation for regulatory reviews.
• Participate in IDG meetings to coordinate care and medication management.
• Monitor pharmacy utilization and appropriate prescribing.
• Collaborate with leadership on policy, compliance, and QAPI initiatives.
• Serve as a community liaison to expand hospice partnerships.
• Support staff and community partner education.
