Job summary

Gentiva Health Services, Inc. is seeking a Hospice Physician to ensure compassionate, goal-directed care for hospice patients while upholding the highest medical standards. You will collaborate with interdisciplinary teams, oversee regulatory compliance, and guide quality improvement efforts to improve patient outcomes.

This is a job notification only; the Hospice Physician will invoice Gentiva for services rendered and may travel as needed to support program growth and patient access to care.

Qualifications

• Doctorate degree from an accredited medical school.
• Board certification in a recognized specialty (preferably primary care).
• Active medical license in the practicing state.
• Valid NPI and DEA registrations.
• Hospice and Palliative Medicine board certification preferred; PECOS/Medicaid as required.
• Ability to travel 15–25% based on program needs.
• CPR and TB clearance required.

Responsibilities

• Certify and recertify hospice eligibility with Medicare compliance.
• Conduct and supervise home visits and complex case consultations.
• Provide 24/7/365 on-call physician coverage coordination.
• Maintain timely medical documentation and support regulatory reviews.
• Participate in IDG meetings to coordinate care and med management.
• Monitor pharmacy utilization and appropriate prescribing.
• Collaborate on policy, quality improvement (QAPI) initiatives.
• Serve as community liaison to enhance hospice visibility.
• Support staff and partner educational programs.
