Job summary

AMEDISYS HOME HEALTH is seeking a hospice physician to ensure high quality of life for patients at end of life, focusing on comfort, dignity and emotional support. You will work with nurses, social workers and chaplains to implement individualized care plans addressing physical, emotional and spiritual needs.

Responsibilities include comprehensive patient assessments, managing symptoms, admissions, certifications and coordination with the care team.

Qualifications

• MD/DO degree from an accredited med school is required.
• Current CPR certification is required.
• Licensed to practice medicine in the state is required.
• Membership on hospital medical staff in good standing is required.

Responsibilities

• Conduct comprehensive, face-to-face assessments with patients and provide palliative care to alleviate pain.
• Oversee admissions, discharges, certifications, and recertifications and manage medications.
• Certify terminal prognosis at admission and at each certification period.
• Participate in interdisciplinary team meetings to update care plans for hospice patients.
• Remain available for acute clinical problems and provide on-call consultations.
• Communicate empathetically with patients, families, and caregivers throughout hospice care.
• Coordinate care with other providers to ensure seamless transitions and continuity.
• Lead goals of care discussions with patients and families as appropriate.
• Educate patients and families about hospice philosophy, treatment options, and end-of-life care.
• Serve as physician or designee of the Professional Advisory Committee.
• Maintain accurate medical records and ensure regulatory compliance.
• Collaborate with attending physicians to determine eligibility and gather records.
• Provide on-call coverage for emergencies and referrals; maintain shared call schedules.
• Conduct quarterly telephone meetings with Hospice Medical Director.
• Supervise Nurse Practitioners per state requirements.

Education

Medical degree (MD or DO) from an accredited medical school

Current CPR certification

Current license to practice medicine in the state

Membership in good standing on the medical staff of at least one contracted hospital
