Job summary

The role supports NP scheduling, patient coordination, and high-quality care in a field-based setting.

As a CMA with 1 year of experience, you will collaborate with physicians and nurses, maintain records, and ensure timely communication across branches, while upholding VIA Health Partners’

Qualifications

• High school diploma preferred, CMA certification required and in good standing with AAMA.
• Minimum 1 year experience; homecare/geriatrics experience preferred.

Responsibilities

• Deliver patient care and initiate contact with patients and families.
• Schedule NP visits and manage weekly NP calendars.
• Coordinate appointments and reminders within geographic area.
• Maintain accurate clinical documentation per VIA Health Partners guidelines.
• Participate in interdisciplinary group conferences and refer needs to team members.
