Job summary

VIA Health Partners delivers comprehensive hospice and palliative care with a patient-centered approach. The CMA-certified clinician initiates contact with patients and families, coordinates visits with NPs, and maintains detailed clinical records in eCW, ensuring timely and compassionate care across the plan of care.

Preferred candidates have at least 1 year of homecare or geriatrics experience, a high school diploma, and strong teamwork skills to support interdisciplinary group conferences and

  • CMA certification required and must be in good standing with AAMA.
  • High level of professionalism and ability to work in a team setting.

Responsibilities

  • Initiates patient/family contact to introduce hospice services and schedules visits.
  • Maintains accurate clinical documentation in the EMR according to VIA Health Partners guidelines.
  • Coordinates with NP and interdisciplinary team to ensure timely patient visits.
  • Manages scheduling and referrals, ensuring capacity optimization and productivity.
  • Supports branch operations and communicates with patients, families, and referral sources.

Reviews clinician schedules daily or more frequently as needed to determine capacity opportunities and ensure productivity standards are met.Ensure clinicians are assigned and scheduled in the most efficient geography, maximizing clinician efficiency, utilizing the lowest possible discipline, matching skill with required care, and optimizing clinician capacity.Ensure missed visits are scheduled by the end of the current week or referred back to leadership for resolution.Works collaboratively with field clinicians to ensure all patient visits are scheduled and completed as ordered within the plan of care.Manage calls from patients and field staff related to scheduling issues. Refers clinical and performance-related issues to the Manager of Practice Operations.Monitors hospitalized patients, communicates, and ensures the team follows up as necessary.Completes requested schedules ensuring all orders and services (i.e. lab draws) are included as required.Supports branch operations and works collaboratively to ensure timely communication with patients, clinicians, referral sources, and other office staff.Maintains medical records, obtains supplemental record and processes document attachment.Runs all applicable reports and responds to workflow taking appropriate actions.Assists internal and external transfer of patients between service lines as appropriate.Maintains the upmost patient experience and connection with their patient panels and NPs they support.Administer virtual visits in the field as appropriate.Quick response times to patients and care team; readily available during business hours.Interdisciplinary GroupAttends and participates in the interdisciplinary group conferences [IDG] and special care conferences.Assumes responsibility for establishing and maintaining effective working relationships with team members.Identifies specific patient/caregiver needs and makes referrals to appropriate team members.DocumentationEnsures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.Professional and Personal AccountabilityDemonstrates competency in knowledge and skills within scope of practice.Ensures that VIA Health Partners professional reputation is maintained and projected.Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Incorporates new information and methods into practice.Maintains current professional licensure/certification, CPR certification and CEU requirements.Maintains productive and professional relationships with external agencies and healthcare providers.Exhibits time management skills. Able to plan workday while remaining able to respond appropriately to unforeseen obstacles.May perform other duties as required.High School Diploma preferred, plus training/certification in related field required.Certified Medical Assistant (CMA) is required and must be in good standing with American Association of Medical Assistants (AAMA).Minimum 1 years’ experience required. Experience in homecare, working with terminally ill and/or geriatrics preferred.
