Job summary

VIA Health Partners, a leader in end-of-life care, is seeking a registered nurse to coordinate and deliver patient/caregiver care in our hospice program. You will conduct nursing assessments, develop and update care plans, and educate patients and families while ensuring continuity of care.

You will communicate with the IDG, be mindful of infection control, and maintain accurate clinical documentation. A compact state license, CPR, and 1–2 years of relevant experience are expected; CHPN is

• RN license in good standing and active in home state or compact state.
• Current CPR certification is required; CHPN preferred.
• Minimum 1 year inpatient (IPU) experience; 2 years for other nursing roles.

Keeps supervisor apprised of any complex patient/caregiver issues.Applies principles of infection control. Ensures all areas of facility are clean and appropriately stocked with supplies. Keeps the family room clean throughout the day. Ensures proper chemicals are used for cleaning different surfaces. Ensures protective equipment is used when working with chemicals, i.e., gloves, glasses.Minimum QualificationsA Diploma or associate degree in nursing is required; Bachelor’s Degree preferred.A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline.Current CPR required; CHPN preferred.Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.Computer proficiency is required
