Job summary

VIA Health Partners in Monroe, NC, is seeking a dedicated Registered Nurse to join our hospice team, delivering compassionate patient and caregiver care in the home or facility setting.

You will collaborate with IDG members, perform nursing assessments, develop and update care plans, and educate families while maintaining detailed documentation and infection control. CHPN preferred and state licensure required.

Qualifications

• Diploma or associate degree in nursing required; Bachelor’s degree preferred.
• Current RN license, multi-state (NLC) preferred or eligible.
• Current CPR certification required; CHPN preferred.
• Minimum 1 year IPU experience and 2 years in other nursing roles.
• Experience with life-limiting illnesses and hospice care.
• Proficient with electronic charting and documentation.

Responsibilities

• Coordinate and deliver patient/caregiver care.
• Conduct nursing assessments and develop care plans.
• Educate patients and families and provide ongoing support.
• Communicate with IDG members and care teams to ensure continuity of care.
• Maintain accurate, timely clinical documentation and compliance with guidelines.
• Apply infection control practices and safety protocols.
• Support crisis response and adapt to changing patient needs.

Ensures all areas of facility are clean and appropriately stocked with supplies. Keeps the family room clean throughout the day. Ensures proper chemicals are used for cleaning different surfaces. Ensures protective equipment is used when working with chemicals, i.e., gloves, glasses.Minimum QualificationsA Diploma or associate degree in nursing is required; Bachelor’s Degree preferred.A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline.Current CPR required; CHPN preferred.Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.Computer proficiency is required.
