Heartland Hospice is seeking a Patient Care Manager to lead clinical operations and ensure high-quality, compliant hospice services. You will oversee patient care, scheduling, triage referrals, and lead Interdisciplinary Group meetings while mentoring staff and ensuring regulatory standards are met.

The role requires an RN license, 3+ years of direct patient care (2+ in hospice), and strong leadership and documentation skills. Local Missouri operations with home-based care support.

Qualificações

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum 3 years of direct patient care experience, including 2 years in hospice or home-based care.

Responsabilidades

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize based on clinical judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead IDG meetings, present updates, review charts, coordinate care plans, document minutes.
