Job summary

Ovation Hospice in Denver, CO is seeking a Patient Advocate (Hospice Liaison) to actively build trust with physicians, discharge teams, SNFs, and case managers. You will serve as the bridge between families and hospice care during emotionally challenging times, driving timely referrals and patient admissions.

Qualifications

• Prior hospice/home health or healthcare liaison experience strongly preferred.
• Ability to build trust quickly in hospitals, SNFs, and families.

Responsibilities

• Build and manage a referral territory with physicians, hospital case managers, and SNF social workers.
• Educate referral sources on Ovation's capabilities and hospice criteria.
• Meet with families to explain hospice benefits and options.
• Drive admissions growth by generating and converting referrals.
• Maintain accurate activity tracking in CRM with visits and pipeline.
• Collaborate with Administrator, clinical team, and intake coordinators for seamless hand-offs.
