Ovation Hospice in Arizona seeks a Patient Advocate to cultivate relationships within the community and educate families on hospice care. This field-based role focuses on building trust with healthcare partners and coordinating patient admissions.

We are looking for Patient Advocates who are driven, self‑directed, and genuinely moved by this work. This is a field‑based relationship role: you will be out in the community every day, building trust with physicians, hospital discharge teams, skilled nursing facilities, and case managers — and serving as the bridge between families navigating an incredibly difficult time and the care they deserve.
This is not a transactional sales position. It requires someone who can hold both the relational and the professional simultaneously — who understands how to build a referral territory from the ground up, stay accountable to their own numbers, and show up with warmth and credibility in environments where the stakes are high.
