Job summary

Ovation Hospice is seeking a Patient Advocate (Hospice Liaison) in Denver to build a referral network and educate healthcare partners about hospice services. This field-based role requires self-direction, relationship-building skills, and a compassionate approach to families navigating serious illness.

The successful candidate will manage a territory, track activities in CRM, and collaborate with clinical and administrative teams to ensure seamless patient intake and timely enrollments.

Qualifications

• Experience in hospice, home health, healthcare sales, case management, or community liaison preferred.
• Strong relationship-building and credibility in diverse settings.
• Ability to manage own territory, track activity, and hit targets.
• Comfort explaining hospice benefits and navigating emotionally complex decisions.
• Commitment to Ovation Hospice mission and patient-centered care.

Responsibilities

• Build and manage a referral territory with physicians, hospital teams, SNF social workers, and partners.
• Educate referral sources on Ovation's capabilities, criteria, and timely hospice referrals.
• Meet with families to explain benefits and options during difficult times.
• Drive admissions growth by generating and converting referrals to patients.
• Maintain accurate CRM activity logs and pipeline with discipline.
• Collaborate with Administrator, clinical team, and intake coordinators for smooth hand-offs.
• Represent Ovation Hospice in the community with professionalism and care.
